I've always seen this kinda metaphorically as he is killing innocence in girls. By sexual means but also in ways like turning them away form being goodie goods and such.

"I kill little girls" = "I break girls' hearts"
It's a warning to the girl to whom the song is addressed, like, "Stay away from me! I'm bad news! I'm a psycho! I hate you! Leave me alone!"
She's trying to get under his skin and change him, i.e. in "The Church Channel," etc.

I always saw this song as sort of an anthem to young teeny bopper girls that bombard musicians.
He has anxiety and doesn't like being around people all of the time and I think girls like those
are the ones that cause the most damage so to speak.
